Buttock Pain: A Practical Approach to Differential Diagnosis
Buttock pain is a common presentation in musculoskeletal practice, but identifying its source can be surprisingly difficult. The lumbar spine, hip, sacroiliac joint, proximal hamstring, and peripheral nerves can all contribute to symptoms in the same region—and pain location alone rarely tells us where the problem is.

Instead, clinicians should first classify the presentation, rule out serious pathology, and then use symptom behaviour and targeted examination findings to narrow the differential.

Start With the Big Picture
A useful way to approach buttock pain is to divide the differential into several broad categories:
☑ Lumbar spine and radiculopathy
☑ Sacroiliac joint pain
☑ Hip joint pathology
☑ Proximal hamstring disorders
☑ Deep gluteal nerve syndrome
☑ Peripheral nerve entrapments
☑ Less common conditions, including ischiofemoral impingement
☑ Red flags and non-mechanical presentations
This classification provides a framework for deciding what to examine next rather than relying on a single provocative test.
Importantly, patients can have more than one contributing problem. For example, lumbar spine involvement and proximal hamstring tendinopathy may coexist.
First: Rule Out What You Don't Want to Miss
Before focusing on musculoskeletal structures, the subjective assessment should screen for red flags.
Cauda equina syndrome requires urgent attention, particularly when buttock or leg symptoms are accompanied by saddle sensory changes or bowel and bladder dysfunction. Other concerning features include unexplained weight loss, fever, general malaise, a history of malignancy, significant night or resting pain, and symptoms that progressively worsen.
Bone stress injuries should also be considered, particularly in athletic populations following a sudden increase in training load. Sacral stress fractures may present with activity-related buttock pain and can sometimes be difficult to identify clinically, making imaging important when uncertainty remains.
The pattern of symptoms can also provide important clues. Mechanical pain generally changes with movement and load, whereas concerning non-mechanical presentations may be less influenced by movement and more progressively worsening.
Look Beyond Pain Location
One of the most useful principles from the lecture is to ask what the pain does, rather than simply where it is.
For example, sitting intolerance can direct the differential towards proximal hamstring or neural involvement. Symptoms extending below the knee, particularly when accompanied by altered sensation, weakness, or paraesthesia, increase suspicion of neural involvement.
Load behaviour is another important discriminator:
◉ Pain with contraction → consider muscle or tendon involvement.
◉ Pain with stretch → consider tendon or neural sensitivity.
◉ Pain with compression → consider an impingement-related mechanism.
◉ Symptoms changing with lumbar movement → consider a lumbar spine driver.
This approach can make the examination much more efficient.
The Lumbar Spine Should Be High on the List
Lumbar spine involvement is presented as the most common cause of buttock pain in the lecture. Importantly, lumbar-related buttock or leg pain does not always occur alongside low back pain, neurological deficits, or abnormal imaging.
The examination can include lumbar flexion, extension, rotation and side gliding, while monitoring for reproduction, reduction, centralization, or peripheralization of symptoms.
A neurological screen—including myotomes, dermatomes and reflexes—can provide additional information, particularly when radicular symptoms are suspected. The Slump test can also help assess neural mechanosensitivity, although the hamstrings are placed under tension during the test, so symptom interpretation is important.
Don't Overdiagnose the SI Joint
SIJ pain can present around the PSIS and buttock and may be aggravated by transitional movements or unilateral loading. However, no single SIJ test is considered sufficiently reliable to make the diagnosis.
Instead, a cluster of provocation tests can be used. The lecture demonstrates the thigh thrust, distraction, compression, sacral thrust and Gaenslen's tests, with multiple positive tests increasing the likelihood of SIJ involvement.
Palpation, motion testing and leg-length assessments should not be treated as definitive diagnostic tools.
Hip Pathology Can Present as Buttock Pain
Although groin pain is a classic presentation of hip pathology, hip disorders can also produce deep buttock, lateral hip or posterior thigh symptoms.
For femoroacetabular impingement (FAI), reduced internal rotation and pain reproduced with flexion, adduction and internal rotation can increase suspicion. However, the FADIR test is sensitive but not specific, meaning a positive result does not automatically establish FAI.
Hip dysplasia presents differently. Rather than restricted movement, clinicians may find excessive range, apprehension and a sense of instability, particularly in younger active patients.
This distinction is clinically useful:
FAI/OA → restriction and painful compression
Dysplasia → excessive movement and apprehension
Proximal Hamstring Pain: Think Load and Compression
Proximal hamstring tendinopathy is an important differential, particularly in runners, but it can also occur in people who do not participate in sport.
Sitting pain is a particularly useful clue. Other aggravating activities may include running, deep squats, lunges, deadlifts and hamstring stretching.
Clinical testing can include palpation around the ischial tuberosity, resisted hamstring contraction and specific provocation tests such as the Puranen-Orava, modified bent-knee stretch and active 30/90-degree tests.
However, a negative palpation finding does not necessarily exclude the tendon as a contributor.
Management should focus on education, load modification and progressive strengthening rather than repeatedly irritating the tendon with aggressive hands-on treatment or stretching.
Keep Neural Entrapments in Perspective
Deep gluteal nerve syndrome—often labelled “piriformis syndrome”—is presented as an uncommon diagnosis and should not become the default explanation for buttock pain.
Typical features include sitting intolerance and symptoms extending into the leg, sometimes with paraesthesia. Importantly, it is primarily a diagnosis of exclusion after lumbar spine, hip and SIJ contributors have been considered.
Other peripheral nerve conditions can mimic more familiar musculoskeletal presentations. Pudendal neuralgia, for example, is more suggestive when sitting pain is accompanied by perineal or genital symptoms, whereas posterior thigh symptoms are more characteristic of deep gluteal nerve involvement.
Cluneal nerve entrapment can similarly mimic lumbar or SIJ pain. Localized or arcing buttock pain around the iliac crest, particularly when aggravated by lumbar extension, may warrant consideration of this diagnosis.
A Simple Clinical Framework
When faced with a patient with buttock pain, consider working through the following sequence:
1. Rule out red flags.
Could this be a serious or non-mechanical condition?
2. Is there neural involvement?
Look for neurological symptoms, sitting intolerance and neural mechanosensitivity.
3. Is it load-dependent?
Does contraction reproduce symptoms? Think muscle or tendon.
4. Does stretch reproduce symptoms?
Consider tendon or neural sensitivity.
5. Is compression provocative?
Consider hip or ischiofemoral impingement.
6. Does lumbar movement change the symptoms?
If yes, the lumbar spine becomes more likely.
7. Confirm before treating.
Use the overall pattern rather than relying on a single positive test.
Clinical Insights
Buttock pain rarely fits neatly into one anatomical structure. A more useful approach is to identify the behaviour of the symptoms and use that information to guide the examination.
The most important clinical lesson is simple: classify first, confirm second, and rehabilitate progressively.
Rather than assuming that every buttock pain presentation is a hamstring problem, SIJ dysfunction, or “piriformis syndrome,” clinicians can use a structured differential to determine which system is most likely contributing—and, equally importantly, which diagnoses need to be excluded or referred.
This mechanism-driven approach can make a complex presentation more manageable and help clinicians avoid both overdiagnosis and unnecessary treatment.
