Episode summary

Why waiting for pain-free isometrics delays eccentric loading after a hamstring strain, and how exercise-specific progression criteria let athletes start sliders and the Nordic hamstring exercise within days of injury.

Jack starts with the conventional model, based on Järvinen and Mendiguchia: isometrics first, then short-length isotonics, with eccentric work only once isometric knee flexion is pain free and within 10% of the other leg. Under time pressure this leaves little exposure to the stimulus that changes the two key modifiable risk factors, short biceps femoris long head fascicles and low eccentric knee flexor strength. The ACU group instead started every athlete on a bilateral 45-degree bridge, 45-degree hip extension and bilateral eccentric slider, progressing each once six to eight reps were done through full range within acceptable pain. In 43 males, eccentric loading began at a median of five days, against 11 days to pain-free isometrics, and about 40% never met the symmetry criterion at all. Load cell data from the "ham sling" setup showed good between-leg symmetry on the eccentric exercises. On the quadrant of doom graph, most athletes moved from short and weak to long and strong, with median return to play clearance at 16 days.
